Continuity of Care Coverage

Community First Health Plans recognizes that potential members may have concerns about changing health plans or the need to change providers due to a provider terminating his/her contract with Community First. Either of these situations may affect the ongoing medical care they are receiving.

“Continuity of Care Coverage” involves any ongoing, active course of treatment, the current phase of which the interruption of service might possibly cause undue harm to the member. If you believe the Continuity of Care Coverage policy may apply to you, please fill out the “Request for Continuity of Care Coverage” form and submit it to Community First as soon as possible prior to your effective date.
